Submitting and tracking a change request: signing in, filling out the form, and checking status afterward.

1. Signing in

The portal only accepts email addresses the Lasso team has approved. Sign-in uses a one-time emailed code.

  1. Open the portal link. If you are not signed in, you land on the sign-in page.
  2. Enter your email address. Click Send me a code.
  3. Enter the 6-digit code from your email (expires in 10 minutes). Click Verify & continue.

Note: no code after a couple of minutes means your email is not approved yet. Contact the Lasso team to be added. Once signed in, your session stays active for 30 days.

2. Submitting a new request

  1. Fill in the form. Fields marked with a red asterisk (*) are required. See the field table below.
  2. Attach a supporting file if useful (max 50 MB). Zip multiple files into one archive first.
  3. Click Submit request.

On submit, a confirmation message shows a reference number. A confirmation email with that same reference number and a status-check link follows. Keep that email.

Type of change requestedPick the closest match: Data dictionary changes, Protocol modifications, New measures or instruments, New instrument scoring or derived field requests, Pipeline versioning, New workflows or data types, Help & Ticket Center Triage lead personnel changes, or Other.
Date of requestDefaults to today. Change it if submitting on someone else's behalf.
Description of requested changeWhat exists now, and what should change.
Purpose and justificationWhy: the goal, benefit, or problem it solves.
Priority levelHigh (critical or deadline-driven), Medium (important, not urgent), or Low (nice to have).
Requested implementation dateOptional. Only if there is a specific deadline.
Impact assessmentFlags whether the change touches Data Access/Query Tools, Training/Compliance Workflows, Security/FISMA Boundaries, UI Consistency, Backend APIs/Data Handling, or an Interconnection Security Agreement. Pick "I don't know" if unsure; the Lasso team will assess it.
AttachmentOptional, up to 50 MB. PDF, Word, Excel, PowerPoint, images, text, CSV, or ZIP. Click Remove to swap a chosen file.

3. Checking on a request

Fastest way

Open the confirmation email and click Check request status. This opens your request directly (signing in again first, if your session expired).

All your requests in one place

Go to the portal and choose My Change Requests. While signed in, it lists every request tied to your email address. Click a request to expand it.

Status meanings

StatusWhat it means
SubmittedReceived. Waiting on a first review.
Needs More InformationThe Lasso team needs something from you before moving forward. Check the comments.
ApprovedApproved. Lasso is working on it.
CompletedThe change has been made.
DeclinedWill not be implemented as submitted. Check the comments for why.
CanceledWithdrawn.

Tip: you can add a comment to any request from the same expanded view.
